How antiarrhythmic Drugs Increase the Rate of Sudden Cardiac Death
نویسنده
چکیده
Two large clinical trials of drugs that exhibited significant antiarrhythmic properties in single cells were found to increase the rate of sudden cardiac death in patients by two to three fold over untreated patients. We hypothesized that premature excitation within the drug-altered vulnerable region, a region that trails each excitation wave, might be one mechanism for initiating re-entrant tachyarrhythmias that could lead to ventricular fibrillation and sudden cardiac death. With numerical studies of a cardiac cell model, we probed the determinants of the vulnerable period. We found that antiarrhythmic drugs that block the sodium channel can increase the duration of the cardiac vulnerable period by both slowing conduction velocity and reducing the gradient of excitability. Coupling the dynamics of drug binding to ion channels with wave formation in a nonlinear excitable medium provides new insights into possible arrhythmogenic mechanisms.

Sudden cardiac death is a significant cause of mortality in the United States, with an estimated 400,000 cases per year (1 1). Discovery of an effective antiarrhythmic drug capable of reducing the incidence of sudden cardiac death by as little as 10% would result in the saving of approximately 40,000 lives annually. Obviously, there is a need for an effective form of therapy capable of safely t...

Survivors of acute myocardial infarction (MI) remain at risk of sudden cardiac death after being discharged from the hospital. In these patients, frequent premature ventricular contractions (PVCs) and depressed ventricular function herald sudden cardiac death.1-3 These observations triggered the idea that if PVCs were suppressed by antiarrhythmic agents, then sudden cardiac death could be avert...

The heart can be considered as a 3D mass of nonlinear excitable medium. With simple models of an excitable cell, one can probe arrhythmogenic processes with numerical experiments and possibly identify new therapeutic strategies. An important target for such experiments is the reduction in the incidence of sudden cardiac death in patients with cardiac disease. Potentially fatal cardiac rhythm di...

Sudden cardiac death usually occurs secondary to a ventricular tachyarrhythmia. Even under ideal circumstances only 20% of patients who have an out-of-hospital cardiac arrest survive to hospital discharge. Therefore, aggressive treatment and screening of high-risk patients are mandatory to improve survival rates. Risk stratification of high-risk patients, such as the post-myocardial infarction ...
